On February 1, 2003, NASA's Space Shuttle Columbia disintegrated upon re-entry to Earth, resulting in the loss of seven astronauts. Similarly, on April 20, 2010, the Deepwater Horizon Oil Rig in the Gulf of Mexico suffered an explosion, which led to the loss of 11 lives, 17 injured, and incurred damages worth $50 billion. These two catastrophic events are significant in modern history and have prompted inquiries into the root causes of such tragedies.
