Ein alter Bekannter von Dr. Watson, der Geschichtsdozent Dr. Irvin Marsh, bittet um Hilfe. Sein guter Freund wurde von einer merkwürdigen Krankheit befallen und liegt nun in einem geistig verwirrten Zustand im Krankenhaus. Doch scheinbar gibt es keine Gemeinsamkeiten zwischen den Männern. Dann wird auch Dr. Irvin Marsh ins Krankenhaus gebracht, auch er ist Opfer der seltsamen Krankheit geworden. Sherlock Holmes und Dr. Watson setzen alles daran, die Ursachen für die merkwürdige Krankheit zu ermitteln, und geraten dabei an gefährliche Gegner.
