1) This 'brilliant doctor' notices a feature of a patients treatment which another doctor has missed, their revelation is ignored demonstrating how arrogant and short-sighted the original doctor is.
2) A patient is perceived as difficult in some way, the 'quick thinking' and 'witty' doctor thinks up a lie with which he can fool the patient so as to avoid embarrassment to themselves.
